Dear Group Administrator:

Thank you for your application to United Concordia for NETFlex group dental coverage through the Northwest Employers Trust.  

Enclosed please find Administrative Procedures Manual for your reference.  This Manual is designed to assist you with daily administration of your plan.  Included in the Administrative Manual are copies of the Certificate of Coverage, Schedule of Benefits and a sample ID Card.  Please take some time to review.  Please note all premium checks are payable to United Concordia. 

Your employees will receive their ID cards within 10 days of completed processing of group information.  Be aware than an ID card is not required in order to obtain dental services.  Until the ID cards arrive, a copy of the employee’s application can be used as a temporary ID card, if needed.  Each employee will also receive a Certificate of Coverage and a Schedule of Dental Benefits.  

Our directory of Participating Dentists can also be accessed online at www.unitedconcordia.com, selecting National-Fee-For-Services Providers.  When using one of these participating providers, members are responsible only for deductible and coinsurance amounts that are part of the plan design; there is no balance billing for covered services.
